Dr Dilawar Singh at the Skin Logics Clinic has a special interest in Botulinum toxin treatments and regularly attends meetings and workshops to keep up with the latest techniques. He performs all Cosmetic treatments himself.  Each procedure is tailored to the individual client requirements.

WHAT IS BOTULINUM TOXIN?

Botulinum toxin is a natural purified protein that relaxes wrinkle-causing muscles creating a smoothed, rejuvenated and more youthful appearance. We use the Dysport brand of Botulinum Toxin at our clinic. Other reliable brands available in the market are Botox and Xeomin. The active ingredient in Dysport is botulinum toxin type A, derived from bacteria in much the same way as penicillin is derived from mould. It is manufactured in sterile laboratory conditions. It is a non-surgical procedure that smoothes the deep, persistent facial lines that develop over time. A single ten-minute treatment relaxes the muscles responsible for those lines to form and keeps them relaxed for about four months.

WHAT DOES TREATMENT WITH BOTULINUM TOXIN INVOLVE?

It is a non-surgical procedure in which a small amount of Botulinum toxin is injected into certain selected facial muscles using a  fine 30/33 G needle. Generally, Botulinum toxin  (Dysport) treatment for the vertical lines that appear between your eyebrows when you frown (glabellar frown lines) takes about 10 minutes. Discomfort is minimal and very short-lived, and no local anaesthetic is usually required. The injection can sting for a few seconds, but usual activities can be resumed soon after. Following the injection, it takes about 2-3 days for Botulinum toxin to begin taking effect and it takes about seven days to see the full effect.

HOW DOES BOTULINUM TOXIN WORK?

The Botulinum toxin blocks signal from the nerves to the muscles. The muscle that is injected can no longer contract, which results in relaxation and softening of the wrinkles the wrinkles. It is most often used on, crow’s feet (lines around the eye), forehead lines and frown lines. It can be used to treat Axillary Hyperhidrosis (problem underarm sweating).

HOW LONG DOES BOTULINUM TOXIN LAST?

Normally, if the right amount of Botulinum toxin is injected by a trained doctor in the right muscles, the results could last 3-4 months. BOTULINUM TOXIN should last longer than a few weeks, but the duration varies. First time Botox users, might experience that the toxin is wearing off in < 3 months.

ARE THERE ANY SIDE EFFECTS?

The side effects are usually localized and temporary to the area of injection. These include mild bruising or soreness, which can easily be covered with makeup. There can be a headache, and less commonly, you may develop a heavy eyelid (ptosis) which may last 1-4 weeks which we may be able to treat with some eye drops. No permanent side effects because of Botulinum toxin have been reported. The toxin should not be used during pregnancy or breastfeeding or for patients with a neuromuscular disease.

WHO SHOULD ADMINISTER BOTULINUM TOXIN?

Botulinum Toxin prescription can only be obtained from a

Registered Doctor with the MCNZ.  It should only be injected under the supervision of a registered medical practitioner. Dr Dilawar Singh at Skin Logics and Hamilton Lake Clinic has taken specialized training in these cosmetic procedures and performs all injections himself – no treatments are delegated to a nurse.

Please beware of “bargain Botox” offers. If the cost seems too cheap, it is likely that the product is diluted or may not be a registered Allergan Botox or Galderma Dysport. Recently, some Chinese and Korean products are being imported into New Zealand. These products do not have a proven safety record.

Injectable Dermal Fillers

Dermal Fillers are a modified form of hyaluronic acid which restores Fulness and Volume to the skin. It smoothes the facial wrinkles and folds, enhances facial shape or to create fuller lips. These products have revolutionised cosmetic medicine over the last ten years. Above all, dermal fillers are non-permanent, safe and can be dissolved if the result is not to your liking.

At Skin logics and Hamilton lake clinic we use JUVEDERM® and RESTYLANE®, both FDA approved medications that will give you instant, long-lasting results (up to 12 months).

What is the constitution of  JUVÉDERM ®?

JUVÉDERM® is an injectable gel that is made from a complex sugar called hyaluronic acid (HA). Hyaluronic acid is found in the tissues of living cells including human skin. It assists in hydration of the skin by attracting and holding water. It also combines with elastin and collagen to help provide skin elasticity and structure. The HA in RESTYLANE® and JUVÉDERM® is not derived from animals. It is produced especially for replacing lost skin volume, smoothing of wrinkles and for rejuvenation of the lips (Lip enhancement).,

HOW WILL I LOOK IMMEDIATELY AFTER TREATMENT?

There can be temporary puffiness of the treated areas. Occasionally, there may be temporary blushing, slight tenderness or bruising around the treated areas. These are all normal and common occurrences and should clear up within a few days.

Where can the dermal fillers be used?

Dermal fillers can be used over the face (and are even used in other parts of the body). However, we at Skin logs do not use the dermal fillers in high-risk areas such as nose and glabellar area. They can be used to effectively treat the following concerns (and more):

  • Nasolabial folds (Dermal Fillers are great for lines or folds from the sides of the nose to the end of the lips)
  • Perioral area (wrinkles around the lips)

  • Hollows under the eyes

  • Marionette lines (These are the lines from the corners of the mouth to the jawline, increasing the downturn of the mouth).

ARE DERMAL FILLERS SUITABLE FOR EVERYONE?

No, hyaluronic acid injections should not be injected into areas where the skin is already inflamed or infected.  Cold sores at the site of injection may reoccur. We usually give you a prescription for anti-viral tablets to prevent this. Dermal filler injections are not suitable for breastfeeding or pregnant women.

DO THE INJECTIONS HURT?

Depending on where the wrinkles or lines are to be filled, the pain will vary. Juvederm and Restylane have a local anaesthetic within the gel. So, the numbness occurs once the needle is under the skin and the injection process is started.  We use a fine 31g needle which is the smallest gauge available on the market. The skin can be anaesthetised before the procedure if the client prefers that. We can also use ice to cool the area down which reduces the pain and decreases the chance of bruising.  We can put in a dental block if someone is feeling pain even after the measures mentioned above. For Most people, these injections are relatively painless or at best a bit stingy.

HOW LONG DOES THE CORRECTION LAST?

Most people need repeat injections after 9-12 months. Over a period, the interval between touch-ups increases and the total amount of filler required to maintain the effect reduces.

WHAT IS THE COST INVOLVED IN A DERMAL FILLER TREATMENT?

ARE DERMAL FILLERS SAFE?

When done by trained professionals, fillers are not only safe, but effective.

Blindness is very rare side effect of Dermal fillers but is worth a mention because of its seriousness. It can occur if the dermal filler is accidentally injected or migrates into a blood vessel supplying the eyes. Since the face is a very vascular structure and is full of arteries, it can happen with injection at any site. Because of this reason, we at Skin Logics do not inject high-risk areas of the face. Careful Injection minimises the risk of serious complications.

On similar lines, the fillers can cause arterial occlusion to some part of the facial skin which can lead to skin damage because of blockage of blood supply. We observe the patients in our clinic for 20-30min after injection to make sure that is not the case. Prompt treatment with Hylase ( An enzyme that breaks down the filler) is effective. We do hold Hylase in stock at our clinic.

The Risk is be minimized with careful injection, good injection technique and knowledge of anatomy.

We don’t recommend the use of any “permanent” fillers in any case, due to the high risk of long-term complications.

Platelet Rich Plasma  (PRP) Skin Rejuvenation

Platelet Rich Plasma (PRP) Skin Rejuvenation is a simple, safe and non-surgical procedure utilising your body’s cells for your skin rejuvenation – no artificial products involved.

HOW DO PLATELETS REJUVENATE SKIN?

Platelets are the blood cells that help the tissue to heal and grow new cells. The PRP, injected into specific areas of the skin, act as a matrix that promotes your collagen to grow, regenerates tissue, and thus acts as to naturally smooth and tighten the skin. In this way, PRP softens wrinkles and improves tone and creating softer skin texture.

WHAT ARE THE USES OF   PLATELET RICH  (PRP)?

PRP has a long history of being used in orthopaedic, dental, and reconstructive surgery and more recently have been applied to dermatology, cosmetic facial, skin wound healing and rejuvenation. WHAT HAPPENS DURING AND AFTER THE PRP SKIN REJUVENATION PROCEDURE?

A small amount of blood will be drawn from your vein. While the PRP is being prepared in a machine called centrifuge, your skin will be cleaned and prepared for the procedure. The PRP injections take a few minutes and are only mildly uncomfortable. The procedure is quick (20 minutes total), relatively painless, and requires no post-procedure care or time off. Mild swelling, bruising or redness fade within 1-3 days.

WHAT RESULTS CAN BE EXPECTED?

The ultimate goal of PRP skin rejuvenation therapy is to tighten skin, improve skin texture and tone, soften lines and pores. Results start showing up 3-4 weeks after the treatment session, and it continues to improve with time. Three treatment sessions are recommended, and they have to be spaced 1-2 months apart, to achieve the best results. PRP treatment can be combined with other treatments like (Botulinum toxin, Fillers, Needling or Laser) to best target a patient’s unique issues and thus achieve the final goal of younger looking skin.

Hyperhidrosis (Excessive Sweating)

Hyperhidrosis (Excessive sweating) affects millions of people around the world and can be managed by targeting the sweat glands with Botulinum toxin.

Hyperhidrosis involves overactive sweat glands which can lead to excessive sweating. This can become embarrassing, stressful uncomfortable, and be disabling. Excessive underarm sweating is one of the most common presentations and this can disrupt many aspects of a person’s life. What is hyperhidrosis?

Hyperhidrosis is a condition that affects millions of people around the world. Uncontrolled sweating can occur in various parts of the body including the hands, feet or underarms. Although high temperatures or stress can increase perspiration, they don’t cause hyperhidrosis. Hyperhidrosis may be inherited trait or physiological condition that stops the body from properly regulating its temperature.

What is Botulinum treatment?

Botulinum Toxin is a natural, purified protein and it temporarily stops the release of a neurotransmitter (chemical) that signals perspiration thus helping to reduce excessive sweating. The Botulinum toxin is derived from a bacterium, similarly as penicillin is derived from mould. It is then manufactured in sterile laboratory conditions.

Botulinum Toxin is a prescription-only medicine and is registered for the treatment of severe axillary hyperhidrosis (excessive underarm sweating).

How does the treatment work?

At Skin Logics Clinic, during your free consultation, our Doctor will determine if Botulinum toxin is a suitable treatment.

Botulinum Toxin Treatment is a non-surgical procedure where a small amount of Botulinum Toxin is administered into the skin of the armpits using a very fine needle. The procedure takes around 30 minutes. Discomfort is typically minimal and brief, and although the injection may sting for a few seconds, normal activity can be resumed soon after.

After the injection, it takes about seven days for the Botulinum Toxin treatment to take full effect.

How long does the treatment last?

After treatment the average person experiences about a 60-80 percent reduction in sweating. The effects of BOTOX® last approximately 6-7 months, however, results vary from person to person.

Micro-Needling infusion therapy ( DermaFrac)

Micro-needling therapy has been around for many years, and various devices have been utilised to inflict micro-injuries on the skin to stimulate collagen production. Initially, skin rolling and skin stamping were the treatments available in NZ, but now the latest and most effective device called Dermafrac has become available in New Zealand.

It is an automatic micro-needling device that has been proven to be as effective as ablative laser treatments such as Fraxel, IPL, laser resurfacing and chemical peels and all with little to no downtime!

It helps the skin texture by improving fine lines, wrinkles and minimising pore size. It can also help by reducing scarring from old acne or a newer wound. LED Light Therapy

LED light therapy augments collagen stimulation, reduces irritation, suppresses inflammation, reduces redness, and also reduces bacteria living under the skin.

It is also possible to get just microdermabrasion or the light therapy by itself.

Radio-Surgery

At the Skin Logics Clinic, we use radiosurgery (Surgitron) to remove benign skin lesions, moles and skin tags. This is a technique using high-frequency radio waves to get rid of the unwanted lesions with minimal damage to the surrounding skin. This resultant wound is very superficial, which heals with very minimal or no scarring.

The mole or skin lesion is taken off at the level of the surrounding skin. The wound is automatically “sealed” during the procedure, and it does not bleed. No stitches are needed. The wound can then be covered by a  dressing; however, if it is on the face, some people leave the wound uncovered for cosmetic reasons.  The area heals over next 7-10 days by forming a light brown scab, which will eventually fall off to reveal pink new skin. It is essential to avoid from scratching or picking the wound during this healing process. The area will continue to mature over the next 6 months, eventually fading to match the surrounding skin.

WHAT IS ELLMAN SURGITRON HIGH RF?

Most electrosurgical units operate at a frequency from 200 to 1000 kHz. The electrodes on these devices become hot and cauterise the tissue they come into contact with, causing heat damage to the surrounding tissue. High-frequency radiosurgery, above 1,500kHz (1.5 MHz), transmits pure radiowaves to the tissue without heating the electrode. Intense Pulsed Light-IPL

IPL is a non-invasive light-based procedure that can be used to treat a wide range of skin conditions without disrupting the surface of the skin. IPL helps to reduce the appearance of pigmented spots, freckles and sun damage, small veins and capillaries, rosacea and hyperpigmentation from acne scarring.

WHAT IS IPL?

IPL treatments usually take about half an hour, there is minimal discomfort (it has been likened to a flicking sensation), and there is no downtime – meaning it can be a lunch-time procedure as people can return to normal activities straight away.

WHAT IS THE TREATMENT LIKE?

IPL consists of five, thirty to forty-five-minute treatments spaced three weeks apart. Intense Pulsed Light is flashed along the skin of the face, and in some instances, neck and chest. Clients describe the sensation as feeling like the snap of a rubber band.

WHAT IS ERYTHEMA, CAN THIS HELP?

Erythema is a flushed red appearance of the skin that initially only occurs following exposure to cold or heated environments which, over time, can become a constant flushed appearance. Typically, our clients see a 50-75% reduction in Erythema after just five treatments.

CAN IPL HELP WITH SUN DAMAGE?

Yes, after a series of treatments there is a visible improvement to sun damaged and photo-aged skin that shows irregular pigmentation, fine lines and freckling from chronic sun exposure.
